Care clusters and packages

What are care clusters?
A care cluster is a grouping of service users of similar clinical needs. There are 20 mental health care clusters (cluster 9 is a blank cluster). A service user is assessed using the Mental Health Clustering Tool and assigned to a care cluster using the pattern of scores obtained in conjunction with a personal assessment.


What are care packages?
Care packages describe the care that service users in each of the care clusters will receive.
The Care Package approach has been designed to create a common language to describe what staff are providing (Interventions) and why they are doing it (Aims). A draft set of care packages have been designed by the Norfolk and Waveney clinical leads of Norfolk and Suffolk NHS Foundation Trust. Each package shows the indicative care that an average service user will receive; some may receive more interventions and others less depending on the individual service users’ needs.
